Application Process

• Accepted volunteers will be limited.
• Fill out, sign and return the application and the hour pledge form upon receipt of these packet guidelines.
• Full time volunteers MUST INCLUDE: application, hour pledge form and Shake-A Leg Consent form upon receipt of these packet guidelines.

IMPORTANT! Please be sure to read the volunteer guidelines (thoroughly) before filling out your paper work to make sure you know the criteria of your selection and deadline dates.

Applicants under 18 years of age MUST have their parent/guardian sign all applicable paperwork.
