Application Process

• Accepted volunteers will be limited.
• Receipt of your application on time does not guarantee acceptance
• The following forms must be downloaded, filled out, signed, and be in our office by October 31st for veteran volunteers and by December 30th for new volunters to be able to be considered for the next camp session.
• Forms must be signed by your parent/guardian if you are under 18 years of age, or by yourself if you are 18 yrs or older

3. Shake-A-Leg Consent Form(ONLY full time volunteers)
4. Hour Pledge Form
5. Background check form (18 years and older, also fill out if you will be 18 by camp date)

We need you also to submit the following together with the above filled out forms.

[ ] 2 letters of recommendation (only for new volunteers)

[ ] If under 18 years old … last report card from school (only for new volunteers)

[ ] Recent photo has to be included.
PREFERABLY EMAILED TO bela.florentin@gmail.com Applications without photos will not be considered.

IMPORTANT! Please be sure to read the volunteer guidelines (thoroughly) before filling out your paper work to make sure you know the criteria of your selection and deadline dates.
